Authorized-to-Move Form
WHY IS THIS FORM REQUIRED? Hawaii State Law (HRS 286-271) requires drivers of vehicles that they do not own to obtain the written permission of the registered owner to move a vehicle between islands.
WHO NEEDS THIS FORM? If you are not the registered owner of the vehicle you plan to drive aboard Hawaii Superferry, you must present a notarized copy of this form at check-in or the vehicle will be denied boarding. (If you are the vehicle’s registered owner, this form is not required.)
- You must have this form even if the vehicle is owned by another family member such as your spouse, parent or child.
- Commercial vehicle owners need this form for each commercial driver and vehicle.
- Drivers of rented vehicles have a special procedure. Check with your rental agency.
WHAT DOES THE REGISTERED VEHICLE OWNER NEED TO DO? The registered owner must complete this form and execute it in the presence of a Notary Public. Extended authorization periods can eliminate the need to get a new form each time vehicle moves. A short authorization period and just one island can limit vehicle movement. We suggest you keep this form in the glove compartment.

- Most bank branches have Notary Publics. By Hawaii law, notarization costs $5.00 with second pages costing $2.50
WHAT DOES THE VEHICLE DRIVER NEED TO DO? The driver must bring the notarized form to the port each time vehicle is moved. We suggest you keep the form in the vehicle’s glove compartment. Unauthorized drivers (not listed on the form) will not be permitted to take vehicle aboard.
